Office Policies

  • Appointments
  • Our office operates by appointment only. In scheduling appointments, it is our intent to see you as soon as possible. Our staff and physicians will make every effort to accommodate urgent add on requests. We allow 15 minutes prior to your appointment to complete any new patient forms. We will make every effort to see you on time at your scheduled visit. For your appointment please bring with you all medical records. Patients are expected to bring these upon arrival to the office the day of the appointment.
  • Cancellations:
  • We reserve your appointment exclusively for you. We would request 48 hours notice for rescheduling and/or cancellation of an appointment so that another patient may be able to utilize this slot.
  • Nurses Phone Calls:
  • If our nurses are available at the time of your call, then your call will be routed to the nurses station. However, messages may be taken as needed for our nurses. Please let the receptionist know where you may be reached, including both day and evening numbers. Your call will be returned at the first opportunity.
  • Prescription Refills:
  • For Refills, Blue Cards, Forms or any other business information that needs to be completed, we ask that you provide at least 24 to 48 hour notice. Due to the large number of request we receive, please make sure you call during regular business hours. Calls after hours are for emergencies only! The staff will submit your request for prescriptions to your physician. Once the refill has been authorized, it will be called to your pharmacy. If you need a refill of your child's ADHD medication, you need to call 72 hours prior to pick up. Physician's signature is required. Please have ID available for pick-up.
  • Testing, Orders, and Results:
  • Testing will not be performed without a physician’s order. You must take your physician’s order with you to the testing site. You will automatically be called with your results. If you have not received a call from our office within 5-7 working days from the date of your test or procedure, please call the office.
  • Non-compliance and Transfers:
  • We value our patients here at Madison Valley Pediatrics. We want to give you the best possible care. As your medical provider, we expect compliance with appointments, procedures, and office policies. To be able to provide continuity of care to your children, the following policies have been set:
  • 1. If you have more than 3 No shows, MVP reserves the right to discharge and /or discontinue care and you will be charged for a missed appointment. fee of $10.00 regular appointments/ $20.00 for same day appointments that are made and not kept.
  • 2. Due to the high volume of patients, decisions to accept your child/children to the practice would be subject to approval once you have moved out of state or transferred to another practice.
  • 3. Payment Plans are offered if you have an account balance. If, however, you do not comply with your arrangements, your account will be sent to collections.
  • 4. Our goal here at Madison Valley Pediatrics is to give your children Great Quality care
  • New Office Policies
  • Courtesy calls are made the day before the scheduled appointment, when possible. If you are unable to be reached, you remain responsible for the appointment.
  • Madison Valley Pediatrics will charge $5.00 for all Blue Cards not obtained on the day of the vaccinations. We require 24-48 hrs for completion.
  • Madison Valley Pediatrics requires 48 hours for all prescription refills or forms that you may need.
  • Madison Valley Pediatrics requests that if you have a preference of physicians that you notify the office staff when scheduling your appointment. There may, however, be times when only one provider is available to see your child. We will do our best to make your appointment with the provider you prefer when possible.
  • Should your account be turned over to collections due to an unpaid balance, there will be a 35% fee added to the bill to cover the cost of the collection agency.
  • Dr's excuses are given only in the event that a patient is seen in the office during the illness. If a patient misses school, and isn't seen in the office, we cannot give an excuse.
  • Well Exam
  • If your child is here for a Well Exam/Physical, this is all we can treat on your child's visit that day. Otherwise your insurance Will Not Pay for all or part of the child visit. You will need to reschedule or change your appointment type if available that day.
